Federal/State Screening Process for bog turtle habitat in Adams, Berks, Bucks, Chester, Cumberland, Delaware, Franklin, Lancaster, Lebanon, Lehigh, Monroe, Montgomery, Northampton, Schuylkill, and York Counties. Fill out this form and submit it to the bog turtle contact for your county (listed in this document) for a Phase I bog turtle habitat survey.
