Jack Adcock died of pneumonia at Leicester Royal Infirmary after a doctor confused him with another youngster who had already been discharged. At an inquest at Leicester Town Hall, paediatric registrar Dr Hadiza Bawa-Garba admitted that she interrupted attempts to resuscitate Jack without checking his identity. She admitted that she was not on top of things after working 12 hours without any break and had recently returned from a 13-month maternity leave. SEE CATERS COPY.
