If you would like to become a Great Ormond Street Hospital Foundation Trust member, please complete this form. You must be aged at least 10 years old and you must live in England or Wales.

Our membership database is held and managed by Great Ormond Street Hospital Children's Charity. Your details are personal information and will be confidentially stored in accordance with the Data Protection Act and only used for appropriate purposes.

Our membership values
Great Ormond Street Hospital is committed to working with others to improve children's health and well-being.
