Transitioning a Special Needs Student Checklist
Before you leave your current duty station:

bullitContact your local special education counselor/director and request assistance as to what should be done before the transition to the new school. Write or contact the special education director for the state that you are going to and request local policies.

bullitSchedule an Annual, Review and Dismissal (ARD) meeting to discuss the progress your child has made since your last Individualized Education Program (IEP) review. Ask for written suggestions that may help your child and the staff at the new school.  

bullitRequest a copy of your child's complete educational record to include a copy of the latest IEP. Hand carry all records, samples of your child's work, and other information related to your child's education.  

bullitContact your local Army Community Service (ACS), Exceptional Family Member (EFMP) Program Coordinator. They can assist you with identifying resources at your new duty station. Fort Knox ACS: 624-6291

bullitBe sure to take any special equipment and refill medication prescriptions that your child may need for the next few months

Most Frequently Asked Questions about Local Schools
1. Can my child attend school on post if I live off post?
No. To be eligible for attendance in the Fort Knox Department of Defense schools, a student must meet the following requirements:
bullitBe a legal resident of the installation, residing in the government quarters of the service member.

bullitAll requests for enrollment based on court-ordered guardianship must be submitted to the FKCS superintendent or his designee. Guardians must obtain approval from the installation Housing office for the student to reside with them in government quarters. The Court Order Guardianship and the Housing Approval Form must be given to the FKCS superintendent for determination of eligibility.

bullitStudents may enroll in the Fort Knox schools when the sponsor's assignment to family quarters will be made within 180 school days. The housing office must provide a written statement. Students will be subject to dis-enrollment unless the sponsor provides proof of occupancy in on-post housing within this 180 day period.

bullitA student may remain enrolled for the remainder of the school year if the family changes conditions (moves off post). However, the family must provide transportation to and from school.

2. Can I choose which off-post school my child will attend?
The off post school districts require a student to attend the school designated for their residence. Even though there are several schools in each district, your child must attend school according to your street address. Most off-post districts do not allow children to move out of their assigned school. Since your street address dictates where your child will attend school, we recommend you shop for a school before you shop for a house.

3. Which school is the best to attend?
Each parent and child must decide which school is best for them. This decision is best made by visiting prospective schools, researching the schools via their web page, and talking to teachers, children, and administrators.

4. How can I research the local schools?
Each school district has a web page where you can research test scores, school safety, teacher-pupil ratios, etc. The majority of students in the Fort Knox area attend these school districts.

5. If I know where I will live, how can I find out which school my child will attend?
You should contact the school district where you reside. If you are not sure which school district you reside in, ask your realtor or contact the School Liaison Officer at (502) 624-2305. The School Liaison Officer can also be reached at email address: Knox.School.Liaison@conus.army.mil

6. Where can I get school calendar information and when does school start?
Each school district posts their calendar information on their web site.

7. At what age can my child start school in Kentucky?
Children must be 5 years old on or before 1 October to start Kindergarten in Kentucky.

8. What information will I need to enroll my child in school?
The following information is needed to enroll a child in both on post and off post schools:

bullitChild's original birth certificate
bullitKentucky Immunization Certificate (see next question for more information)
bullitChild's social security card
bullitPhysical exams are required for the 4 year old program, Kindergarten, first grade, and Sixth grade students and any student entering Kentucky schools for the first time
bullitA child entering school for the first time (4 year old program or Kindergarten) must have a vision screening

If you are entering school mid-year, we recommend you provide samples of your child's school work from your most recent school. Also, a copy of any report cards, current IEP, and standardized test scores if possible. This information will help your new school determine the best placement for your child.

9. What are the health requirements for school enrollment?
Vision Exam - All children entering school for the first time (4 year old program or Kindergarten) must have a vision exam by an optometrist or ophthalmologist. Exams are offered by off post doctors only. Contact TRICARE to obtain a list of participating providers.

Immunization Certificate - All students must have a Kentucky immunization certificate that includes screening for a TB Skin test. The immunization certificate is due the first day of school. To receive a certificate you may walk in the Immunization Clinic at Ireland Army Community Hospital without an appointment. The Immunization Clinic is located in the basement of the hospital, room NBF-C2 - across the hall from Primary Clinic # 1. Their office hours are 0730 - 1130 and 1300 - 1500 Monday - Friday. For additional information, call (502) 624-9920. Civilians should contact their local doctor or a Kentucky Health Clinic for this certificate.

Other Immunizations - Students entering sixth grade must have two doses of hepatitis B vaccine. Contact the immunization clinic for more information about the hepatitis B vaccine series.

School Physicals - Students entering school for the first time in either the 4 year old program or Kindergarten must have a physical exam report on file at school before he/she can begin school. Physicals must be dated no earlier than 12 months before the first day of school.
Students entering the sixth grade must also have a school physical. Sixth grade physicals must be dated no earlier than 12 months prior entering sixth grade. Students entering school for the first time in Kentucky regardless of grade must also have a physical. Sponsors should contact their primary care clinic for a student physical appointment.

10. Do the schools offer a pre-school program?
The Fort Knox Community Schools offers a program for 4 years olds. All children who are 4 years old before 1 October and reside in housing on the installation are eligible to enroll in the program. The same eligibility requirements and enrollment information is required. Many off post schools offer a program for 4 year olds if the family meets income eligibility and/or if the child has a learning delay. Parents should contact their school district for more information.

11. What are the requirements if I want to home-school my child?
If a parent who resides on Fort Knox chooses to home school their child, they are subject to both the Department of Defense Education Activity (DoDEA) and to the Kentucky Department of Education home school requirements. Requirements include:
bullitNotify the local school superintendent each year by letter that you have established a home school and report the names, ages and place of residence for the home schooled children.
bullitTeach "those subjects that will educate children to be intelligent citizens". These subjects should include reading, writing, grammar, history, math, and civics. It is recommended that a portfolio of each student's best work in several course areas be kept each year.
bullitMaintain a record of courses taken and grades received for each course.
bullitProvide instruction for no less then the equivalent of 175 six (6) hour instructional days for a total of 1,050 hours of instruction each year.
bullitKeep accurate attendance records of pupil attendance. Attendance records can be kept in a notebook, on a computer, or in another manner that is readily available in case of an inquiry.

12. What services are available for my home school child?
A home school student can receive auxiliary services from the Fort Knox Community Schools if they provide the same proof of eligibility as students who enroll in the Fort Knox schools (reside on the installation). Auxiliary services include the use of academic resources such as textbooks, workbooks, library books, standardized tests, software, and internet access through school media centers. Home school students may participate in music, sports and interscholastic activities that occur between schools. Home school children may receive ESL services, Special Education services, and Gifted Education services. They can take a single or multiple classes at their neighborhood school. Services for home school children living off the installation vary from school to school. Parents must contact their neighborhood school for information.

13. How can I access these services for my home school child?
Parents should contact their neighborhood school principal for information and services offered. Information can be found at the school web site or by contacting the School Liaison Officer at (502) 624-2305.

15. Can my child receive information about Fort Knox prior to our arrival?
Yes. Fort Knox Child and Youth Services (CYS) offers a Student Ambassador Sponsorship program for children/youth. If your child in grades 6 through 12 would like to be contacted by a Student Ambassador, please visit www.knoxmwr.com and click on the Child and Youth Services, School Liaison, youth sponsorship link. Complete the sponsorship form and send to the School Liaison Office. A student ambassador will contact your child as soon as possible. If your younger child would like information, complete the sponsorship form and a CYS staff member will contact you.

16. Are there enrichment and sports programs for my child outside of school?
Yes. Visit www.knoxmwr.com and click on the Child and Youth Services (CYS) link for more information about clubs, sports, and other activities for children ages 6 weeks through senior in high school. Active duty soldiers, Reserve, Guard, retirees, DOD civilians, and Fort Knox contractors are eligible to use CYS programs.

Fort Knox In and Out Processing Procedures
Per AR 60-8-101, all soldiers who have children 0 - 18 years of age must in and out process through Child and Youth Services/School Liaison Services (CYS/SLS).  The CYS/SLS office is located in the basement of Bldg 1384, Vine Grove Road, (502) 624-2305 or 624-6703.

In-processing

(1)   The Adjutant General (AG) office will inform soldiers of the requirement to in-process through Child and Youth Services/School Liaison Services (CYS/SLS) located in Building 1384, Vine Grove Road (One-stop Processing center).  All soldiers with children ages 0 to 18 years are required to in and out process through CYS/SLS.

(2)   AG will provide DA Form 5123-1, In-processing Personnel Record, to each soldier.

(3)   CYS/SLS staff will provide soldiers information concerning local schools, child care options, and youth programs.  Staff will sign soldier's in-processing papers. 

Out-processing

(1)   AG will notify Soldiers of the requirement to Out-process through CYS/SLS at the time of their levy briefing or, if an Officer or NCO, when they submit their Request for Orders (RFO).  The soldier has no more than 10 days to clear all required agencies on the installation.

(2)   The AG office will provide the following forms to soldiers:

a.       DA Form 137-2, Installation Clearance Record

b.      Student Withdrawal form 

(3)   Soldiers will be instructed to take the Student Withdrawal form to the public school which their child last attended. School personnel will review student's records to ensure that all fees have been paid and that any outstanding material has been returned.  School personnel will sign the withdrawal form and return form to Soldier.  

(4)   Soldier will take the signed withdrawal form to the SLS office located in building 1384, Vine Grove Road (One-stop processing center).  A CYS/SLS staff member will sign and date the soldier's Installation Clearance Record and provides information and transitional assistance if needed.  Parents of home schooled children must provide proof of their home school status in order for CYS/SLS staff to sign clearance record.  Soldiers clear all CYS programs at this location.

Fort Knox Partnership in Education (PIE) Mentor Program
Goal: Maximize opportunities for educational enrichment and academic success of children and youth by utilizing the talents and resources of military personnel, civilian employees, and family members.

Concept: Coordinate with the Fort Knox Community Schools and military units/organizations to develop a mentor program that reinforces and supports the school curriculum. The mentor program would enhance the partnership between our local schools, the military community, families, and students. Mentoring can involve one-to-one work with an individual student and can also involve sharing with a group of students about a particular hobby, talent, work-related skill, or military travel experience.

bullitParent Orientations - Orientations for new parents are held every Monday at the Child Development center located on Bullion Blvd in Bldg 4249 and 4250.  Hourly care patrons meet at 1030; Full day and Part day patrons meet at 1130.
bullitParent Advisory Board - The Parent Advisory Board (PAB) meets every third Tuesday at the Child Development center from 1130 - 1300.  The PAB is designed to give parents an active voice in the policies and operation of the CDC.  All parents are welcome to attend the meetings.
bullit"My Buddy" Program - Children new to the CDC are welcomed into the program through a buddy system within each classroom.  Children are made to feel welcome with the help of their "buddy" who introduces them to staff, other children and activities.

bullitSaying Hello and Goodbye to Our Friends - The SAS 4-H Club sponsors Welcome parties for new children coming into Ft Knox.  Flyers on dates, location and time are distributed to the elementary schools.  When our friends are leaving Ft Knox they are given a back pack with the program name as a going away gift.  Inside the back pack is information for parents on helping children cope with relocation and activities for the children to have something to do on a long trip.

bullitParent Dinners - School Age Services sponsors parent dinner meetings.  Families are encouraged to attend and enjoy a nutritious meal with their children and hear what is new in the before and after school program.

Youth Sponsorship Club - Youth Sponsorship club meets the third Wednesday of every month at the Devers Youth Center.  Club members are available to provide tours of the youth center, explain the policies of the facility, and introduce students to the YS staff and other youth.
bullitYouth Sponsorship Tours - Tours are held the first Saturday of each month.  New students can tour the popular teen hangouts on Fort Knox and in Radcliff and Elizabethtown.  The tour includes places such as the local movie theaters, Skate Park, bowling alleys, mall and skating rink.  Call the youth center for more information.
bullitSports Deadlines - Newly arrived youth who have missed the sign up deadline for sports will be added to teams if possible.  Parents need to provide a copy of their PCS orders to the Central Registration office located in Bldg 1384, 624-6703.  The Youth Sports staff will make every effort to place a child on a team.

bullitNew Military Spouse Network - Meets the fourth Wednesday of the month at Army Community Service Bldg 77, Binter Street from 1000 - 1200.  The meetings are held to provide information and provide an opportunity to socialize with spouses new to the military.
bullitDiscover Your Neighborhood Tours - This tour is designed to introduce service members and families to the recreational and historical sights in the local area.  Destinations are within a two hour travel distance from Fort Knox.  Tours are offered bimonthly from 0800 - 1800 and are free to military ID card holders.  Call ACS for tour dates and locations.
bullitNewcomers Orientation - Held every Tuesday from 0900 - 1630 at ACS, Bldg 77, Binter Street.  This two day briefing provides a host of information pertinent to soldiers and families assigned to Fort Knox.  A bus tour of Fort Knox and local community is included in the orientation.  This orientation is mandatory for newly arrived soldiers 03/E7 and below.  Family members are encouraged to attend.
